A new entry will appear in the register when the box is ticked in a Personal Injury that a “Incident & investigation report is required”
– It will automatically populate the date of the incident.
– Select the person who is going to complete the investigation from the drop-down list of names.
– Select a Required Completion Date.
The details of the personal injury are automatically populated.

To add any Training Details required select Add & complete the details:

Subject: type in reason for training
Instructor: type in name of the trainer
Training Type: Select from drop down list
Name of Trainee: Select from drop down list
Duration: Select time required to complete the training
Training Provided by: Select from drop down list of third parties
Description: type in what the training will cover
Date: Select the date of when the training will occur.
Then select Save.(All details will be saved in the Training register)

You can then enter any note relating to the training details directly below the grid.

To add any Plant to the Plant & Equipment Repair Register select Add.

Complete the following details on the pop up screen:
Plant & Equipment: Select item from drop-down list.
The description will self populate
Staff: Select from drop down list who will be responsible for getting it repaired.
Repair Date: Select from date picker the expected repair date.
Name of Repairer: Select from the drop-down list the name of the repairer.
Fault/Damage: Type in the description of the fault or damage.
Repairs Required: type in what repairs are required
Then select Save.(All details will be saved in the Plant & Equipment Repair register).

You can then enter any notes relating to the required repairs directly below the grid.

To add any Plant to the Plant & Equipment Maintenance Register select Add.

Complete the following details on the pop up screen:
Plant & Equipment: Select item from drop-down list.
The description will auto populate
Select whether it will be completed by Staff or Third Party.
Staff: Select from drop down list who will be responsible for completing the maintenance (if completed internally).
License Qualification: If required, select applicable license.
Third Party: If maintenance is going to be completed by a third party, then select their name from the drop-down list.
Maintenance Description: Type in the maintenance required.
Next Service Date: Select from date picker.
Service Interval: Select from drop-down list.
Completed Date: will be entered on the mobile app.
Then select Save.(All details will be saved in the Plant & Equipment Maintenance register)

You can then enter any notes relating to the required maintenance directly below the grid.

To add any identified issues to the Corrective Action Register select Add.

Complete the following details on the pop up screen:
Corrective Action Number: Automatically adds the next number.
Type of Action: Select the required action from the drop-down list.
Reason: Select the required reason from the drop-down list.
Raised By: Select the person raising the corrective action from the drop-down list.
Assigned To: Select the name of the person required to complete the corrective action from the drop-down list.
Date Identified: Select the date the action was identified from the date picker.
Estimated Closure Date: Select the date the action should be completed by from the date picker.
Description: Enter a brief description of the issue.
Proposed Immediate Action: Enter a brief description of actions required to rectify the issue.
Then select Save.(All details will be saved in the Corrective Action register)


If Safe Work/Work Safe have been notified of this accident, please tick the tick box provided.

The next step is look at other factors that may have assisted in creating the accident:

Place a tick in the tick box of one or more of the headings provided – Design, Behaviour, Environment, Management.

Investigators Conclusions: Please complete the text field with a summary of ideas that lead to the accident occurring and actions to be taken to ensure it does not occur again.

Investigator to sign off investigation report via their Safetyminder mobile App.

Office/Manager to sign off the investigation report via their Safetyminder Mobile App.

To load a document select +ADD FILE.

When finished Select Save.
